After Hours Emergency Care

We have a provider on-call 24 hours a day,7 days a week.

If you need to contact us for an urgent problem during non-working hours, simply call the office number and you will be directed to our answering service. Your call will then be returned shortly.

If you feel your need is an EMERGENCY, you should CALL 911 immediately.

About

We are a Patient Centered Medical Home (PCMH). This means our staff and the Primary Care Provider (doctor, nurse practitioner, or physician assistant) are on your team and will help you meet your healthcare needs. Think of us as your “home base” for care. We will do our best to make sure you get the attention you need to stay healthy and get you feeling better when you are sick. Please call us before seeking care elsewhere and we will see you in our office or work with the specialist you need to see.

We will help you set goals to improve your health and support you in meeting those goals. If you need extra help we will assist you in finding it. We are a team and you are a key player.

Office Responsibilities

As your Primary Care Office, we will:

  • Provide you with a health care team who will know you and your family
  • Respect you as an individual, respect your privacy, and listen to you
  • Take care of any short-term illness, long-term illness, and give advice to help you stay healthy
  • Provide you with high quality and safe care based on accepted medical research (also called evidence-based care)
  • Schedule appointments with specialists when you need them
  • Share medical information with other care providers, such as specialists, to coordinate your care
  • Have a healthcare provider on call for your urgent needs,24 hours per day, 7 days per week
  • Call you or mail you about test results promptly
  • Explain your health in a way that you understand and support you in the self-management of your health
  • Help you make the best decisions for your care and take better care of yourself
  • Give you information about classes, support groups, or other services that can help you learn more about your health

Patient Responsibilities

We trust you, as our patient, to:

  • Understand your health: ask questions about your care and tell us when you don’t understand
  • Come to each visit with any updates on your health, medications, and questions
  • Take an active role in planning your care
  • Follow the treatment care plan that we have agreed upon – and let us know if you need extra help
  • Take medications as instructed
  • Keep scheduled appointments or call to reschedule or cancel as early as possible
  • Let us know when you see other health care providers so we can work with them to coordinate your care
  • Call if you do not get your test results within a week
  • In non-emergencies, call us before going to the urgent care or emergency room as we may be able to see you in the office that day
  • Give us feedback to help us improve your care
  • Learn about wellness and how to prevent disease
  • Respect us as individuals and partners in your care
  • Learn about your insurance so you know what is covered
  • Pay your share of the visit fee when you are seen in the office
